This assessment includes:

  2. An in-person testing session to measure cognitive skills.

  3. The completion of rating scales to determine an individual’s ability to participate in daily life activities.

  4. A feedback meeting, which can be done in person or virtually, to review results and discuss supports.

  5. A comprehensive written report that can be used as part of the PDD/AISH application.
